2010 Wellness Challenge: Battle of the Office Bulge
I agree to participate fully in KSBY’s 2010 Wellness Challenge: Battle of the Office Bulge contest, which includes completing a medical history form and following the workout and nutrition program.
I am over 18 years of age.
KSBY Communications reserves the right at its sole discretion to disqualify anyone who violates the rules of the program. KSBY and its sponsors are not liable for any injury or medical condition incurred from participation. This program is not officially supervised by a medical professional.
Do not participate if you are pregnant or if weight loss is not appropriate for you. Always contact your doctor before starting any weight loss program. Even if some weight loss is appropriate for you, please do not engage in excessive weight loss. Again, make sure your doctor agrees with your weight loss plan.
In return for my participation, I will be eligible for special privileges and prizes during the contest.
I hereby irrevocably consent to and authorize the use and reproduction by KSBY Communications Inc, all photography you have and or will take of me during my participation in the 2010 Wellness Challenge: Battle of the Office Bulge. I fully understand that such photographs may be used in the future. All images will be the sole property of KSBY Communications Inc.
Further, I understand that I am not, nor is any heir of mine entitled to any compensation now or in the future for any and all use of said photography.

•Contest is nominate a group of four (4) individuals from a local business to participate in the 2010 Wellness Challenge: Battle of the Office Bulge. The business and participants must reside in San Luis Obispo or Santa Barbara counties. All nominees in each group must work for the same business.
•One entry per person: Determined per e-mail address, per computer (i.e. one computer or Internet connection usually equals one IP address).
•Contest begins and runs daily starting August 19, 2010. Entries will be accepted through Tuesday, August 31, 2010 at midnight. Winners will be qualified the first week of September.
•No purchase necessary.
•To be eligible to play, all qualifiers must meet the following requirements:
oAll nominees must provide their name, address, phone number and valid e-mail address.
oPlayers must be 18 years or older.
oPlayers must live in San Luis Obispo or Santa Barbara counties.
oPlayers must hold a valid drivers license issued by the State of California, or a valid form of ID if you do not have a drivers license.
oPlayers must be United States Citizens or legal residents.
oPrize is non-transferable. Prize has no cash equivalent. Prize may not be exchanged.
oKSBY will be the sole and final judge of eligibility and winner verification.
oWinners name, voice and likeness may be used for publicity, promotion and/or news purposes without further compensation.
oBy acceptance of prize, winner assumes full responsibility for all applicable income taxes, sales taxes, license, and other fees that may apply.
•A KSBY and sponsor committee will choose three (3) teams of four (4) nominees to participate in the Battle of the Office Bulge contest. The winners will be notified by e-mail. If a winner is unreachable after two (2) days, or if not medically cleared to participate, or if that winner is unavailable for prize fulfillment, an alternate winner will be selected. If KSBY cannot find an eligible winner for the prize, that prize will not be awarded. All entrants may be e-mailed announcing the winner.
•KSBY reserves the right to substitute any prize with another prize of equal or greater value.
•Entrants grant KSBY the right to use and publish their proper name and state on air, online and in print, or any other media, in connection with the contest.
•KSBY reserves the right to use any and all information and media related to the contest, including information on contestants obtained through the contest, on air or online, for marketing purposes or any other purpose, unless prohibited by law.
•Employees of KSBY and any participating sponsors, affiliates, their dependents and immediate families, as well as employees of other television stations are not eligible to participate or win.
•All entries become property of KSBY and will not be returned. False or deceptive entries or acts will render entrants ineligible.
•Participants waive any and all claims against KSBY and their affiliated companies and employees for injuries, damages or losses related to the contest.
•By entering, participants agree to be bound by these official rules and the decisions the sponsor, which shall be final in all matters pertaining to the contest. The sponsor may deem an entrant ineligible to participate in a contest if the entrant disregards the rules.
•The sponsor(s), its officers, agents, and employees are not responsible for and shall not be liable for:(1) garbled, illegible, incomplete, damaged, misdirected, late, lost, or failed entries; (2) telephone, electronic, decoder, hardware or software program, network, Internet, or computer malfunctions, failures or difficulties; (3) any condition caused by events beyond the control of the sponsor(s) which may cause the contest to be disrupted or corrupted; (4) any losses or damages of any kind caused by the prize or resulting from acceptance or use of the prize or resulting from acceptance or use of the prize, or from participation in the contest; (5) any printing or typographical errors in any materials associated with the promotion; or (6) use of person’s name, likeness, or voice. The sponsor(s) reserves the right, in its sole and absolute discretion, to suspend or cancel the promotion should virus, bugs, or other causes beyond the control of the sponsor(s) corrupt the administration, security, or proper play of the offering. The sponsor(s) further reserves the right to create a database to utilize the name, street address and e-mail address of an entrant for marketing or other purposes. Entrants may opt out of this database by calling KSBY 6. California law shall apply to this promotion. Any claims must be resolved in the state of California.
•Winner certifies he or she has read, understands and will abide by all contest rules.
